    Horizon Gardens AFH is family owned and operated by Norma and Melanie Burnam. They believe that care giving is what an Adult Family Home is about. They work closely with each resident and their family to individualize care and maximize independence. Norma is a Certified Nursing Assistant with offer five years of experience providing quality care in a nursing home setting. Most recently working as a rehabilitation aide She has also worked as an activities coordinator and loves to cook and entertain. Norma is a very compassionate and competent healthcare professional. Melanie has been caring for the elderly for over twenty years. Fourteen years ago she became a Registered Nurse and has specialized in the field of geriatrics and wound care. She has been the Director of Nursing at several different rehabilitation facilities over the years. Horizon Gardens is dedicated to providing excellent individualized care in an atmosphere where residents are able to maximize and maintain high levels of physical and mental functioning through good nutrition, daily activities and managed care. Residents will have all the modern convinces of home with quality and safety provided by 24hr caregivers. Our staff will provide, accommodate and /or arrange services based on each individuals needs desires and interests. It is the perfect alternative when your loved one can no safely live in their own home . Horizon Gardens is a beautiful single level home located in the Kent Valley, which features a spacious living room and kitchen. 4 private and 1 semi private bedrooms, 3 wheelchair accessible bathrooms. The living areas are tastefully decorated in warm and inviting décor. There is a secure private patio to enjoy the out doors. We encourage residents to bring their own furnishings and decorate their rooms as they desire . We have an open door policy, where family and friends are encouraged to visit often and are always welcome to participate in the activities, outings, share a meal or have afternoon tea on our patio.
