Diminished abilities or stage of disease that would result in a transfer or discharge:
If a patient became combative and disruptive to the other residents we would have to discharge. Our goal is to allow residents to remain in place as long as possible (we do Hospice care). Should a resident need medical treatment above what we can do with assistance from Home Health teams it would also require a transfer to a Skilled Nursing Facility.
