    At Churchville Meadows we provide a wonderful alternative to institutionalization. As a licensed assisted living facility, we provide an RN on call for 24 hours a day, as well as a resident manager available 24 hours a day. Each potential resident is evaluated by an RN to determine the level of care needed.
    All residents are provided with services within the Churchville Meadows guidelines, to meet their individual needs. As a level 2 facility, our residents will be assisted by trained staff able to recognize and accurately describe and define a resident’s health condition and identify likely causes and risks associated with the residents’ condition. We provide and ensure access to necessary health services and interventions. We will provide functional support with some of the activities of Daily Living.

    Our staff has the ability to provide and ensure assistance with taking medication and treatment, including monitoring the effects of medications and treatment. Our staff shall have the ability to monitor and provide intervention for behavioral, psychological and psychiatric support. We have an ongoing diversified activities program to provide the residents with social and recreational needs.
