    Swanber Adult Foster Care has three locations. A male home and female home in the country which offers a very relaxing environment and lots of grounds to relax on. There is also a third female location in the city which is close to public transportation. Hope network transports to all three locations so that consumers may work a job or just attend day programming. We look forward to meeting you!!

    Eligibility requirements:
    The only requirement made by Swanberg AFC is that you are fully responsible for a complete 30 day notice. If you are to move before the 30 days are up you are still financially responsible to the AFC for payment.

    Staff certifications and training:
    Our Specialized Adult Foster Care homes have staff at all times that is trained by licensing and the county (CPR, Passing Medications, Ect...). We are not a nursing facility, but we do a great deal for those that live in our homes. Staff does all transportation to and from Dr. Appointments even though we do not offer any doctors on site.

    We would discharge as soon as we felt we could no longer keep the consumer safe in their home environment. If the consumer begins to wander away from the home, or they are becoming physically abusive.
    Special care programs offered for Alzheimer's and Dementia residents:
    Our consumers that have Alzheimer's currently attend Caretree. This is a day program in the Grand Rapids area. They help with many specific needs of our consumers.
