    St. Raphael’s Health and Rehabilitation (SRHRC) is a skilled nursing long term care and assisted living facility located in rural Northeastern Minnesota, approximately 60 miles north of Duluth, MN. We are part of the Iron Range and are a mining community. SRHRC has 90 skilled nursing beds, located on two floors. There are two wings with one central nursing station on each floor. The assisted living facility is located in the Resident Level; it has eight rooms, is licensed for 14 residents and has one centrally located nursing office. SRHRC provides 24 hour skilled nursing care, short term rehabilitation with Physical, Occupational and Speech Therapy, complex medical care, Palliative care, Hospice and supportive living. Out patient therapy services are offered to the community as well. Other major health services SRHRC provides include: IV Therapy, religious services, Social Services, Family council, Resident council and a variety of therapeutic recreation activities and special dietary services. Mission: St. Raphael’s Health and Rehabilitation Center, a member of the Benedictine Health System, is entrusted with the healthcare ministry of the Benedictine Sisters of Duluth. We are committed to witness God’s love for all people, with special concern for the poor and the powerless, in a compassionate environment which supports individual self-worth in the communities we serve. Vision: St.Raphael’s Health and Rehabilitation Center is based on respect and dignity for every individual. We are a Christian Service provider of nursing care, assisted living and related health care services. We are committed to offering quality care in a homelike environment while supporting dignity and uniqueness of each individual. Our Mission and Vision are communicated across the organization by being displayed in the front entrance, in our break room, through our orientation process for new employees, at employee and resident meetings, and in our marketing materials, such as brochures. We communicate our mission and values through a strong service commitment to the surrounding community, by offering a continuum of care, including assisted living, hospice care and outpatient therapy services, by creating an environment of respect dignity and hospitality, by offering privacy and choice for the people who live here, meeting special care needs of persons with dementia, by striving to be the facility of choice and employer of choice in Eveleth. An annual training on Customer Service Training is also provided to all employees with the focus on our 4 core values: Hospitality, Justice, Respect and Stewardship.
