    Highlights
    Wellspring Health Center is located on the beautiful campus of Evergreen Retirement Community. Wellspring offers skilled nursing and a comprehensive rehabilitation program, which includes physical therapy, occupational therapy, speech/language therapy and nursing services. We offer short-term therapy services following an acute care hospitalization, respite stays, and residential care. We can offer complete continuum of care within our community, including Assisted Living Memory Care and Skilled Nursing Care. Residents participate in our award winning FIT FOR LIFE wellness program which incorporates all aspects of health.
