    Concordia Lutheran Ministries is a faith based community that includes Independent Living, Assisted Living, Skilled Nursing, Concordia Visiting Nurses, Hospice Services, Rehabilitation Facilities including a Hydroworx Pool, Providence Pharmacy, Concordia Medical Equipment and Adult Day Services. We have doctor’s suites on campus including a diagnostic suite. Located 25 miles from Pittsburgh, PA in Butler County.
