Diminished abilities or stage of disease that would result in a transfer or discharge:
At this point, the diminished abilities or changes in condition (medical or behavioral) that would warrant a discharge of a client with Alzheimer's disease/dementia would be excessive wandering/exit seeking especially at night, because at this time we are not a 24 hour awake home, however, this could change at a later time.
Special care programs offered for Alzheimer's and Dementia residents:
We work our activities to the needs and likes of our clients. At this time we have table cards that we place at the meal times that ask questions to get them thinking and it offers the answers on the other side. We also have "current events" were we read and discuss issues happening today. We are pretty much open to new ideas on activities that will help each individual client.
