    We are dedicated to Memory Care. It's our passion to provide the opportunity for a great quality of life by preserving and honoring the essence of the human being in our care.     Diminished abilities or stage of disease that would result in a transfer or discharge:
    The most likely scenario for discharge would be if the individual exceeds our ability to meet their needs as outlined in Title 22. Sometimes, if the individual is a threat to themselves or others, and the individual is not able to be redirected this can result in discharge as well. In both cases, the community will work closely with the family to locate alternate placement.
