    Small, intimate, bed n breakfast-like for 18 residents. Staff is caring and pays attention to each individual’s special needs. There is less distance to walk from room to dining room and lots of hands-on for ADL’s – close supervision.

    Eligibility requirements:
    All ages welcome. Personal evaluation of resident. Completed Medical Evaluation – DSS3122 signed by MD within 30 days of admission. PRN form (as needed meds). Mental Health Evaluation Form, PPD & immunization form, Private Pay, sliding scale for 1 bed, Aids in Attendance, 30 day admission agreement.

More than 1 care attendant’s assistance regularly needed to complete ADL’s. Unable to eat independently – needs to be fed to maintain nutritional status. Exhibits excessive violent or abusive behavior. Unmanageable incontinence, will not allow staff to assist. Drug or alcohol dependence. Immobile. Diagnosed with communicable disease. Failure to pay.

    Gerald Levine Center for Meory Care at Loudonville Home - a secure unit - 16 private rooms.
