Diminished abilities or stage of disease that would result in a transfer or discharge:
A criterion for admission to the special units requires that an individual is able to benefit from specialized programming that supports intellectual loss from a variety of cognitive conditions. Discharge from a special unit is planned with family and or responsible parties when an individual has declined to the point of no longer being able to benefit from the special programming provided. At that point plans for discharge into the main house of the facility or consideration of an alternative service is addressed.
Special care programs offered for Alzheimer's and Dementia residents:
Altercare embraces a philosophy of care based on the most current research and knowledge of dementing disorders. Integration of 57 components of care delivery, specifically established for the cognitively impaired is utilized.
