Upon admission to Fair Oaks Rehabilitation and Healthcare Center, our residents receive a thorough evaluation with individual, rehabilitative goals outlined for the future. Dedicated staff members, including around the clock nursing staff, social services, dietary manager, activity professionals, and physical, occupational, and speech therapists all work together to create a nurturing environment of healing, growth and hope. Coming to Fair Oaks is a new beginning. Fair Oaks Rehabilitation and Healthcare Center is committed to providing our residents, family, and friends with an exceptional level of care and service. We accept many insurance providers as well as private funds and we can help you figure out which financial route is best for you. Amenities available at our facility include Private rehabilitation suites, a beautiful, enclosed courtyard, a Beauty Salon, an On-Site Physicians Program, 24-hour admissions, social and lifestyle events, and much more. In addition to skilled nursing care, rehabilitation services, respite care, and hospice care, Fair Oaks Rehabilitation and Healthcare Center in South Beloit is now home to “The Oaks Outpatient Therapy Center”. “The Oaks” is more than just an outpatient clinic. It is a special place for personal, one-on-one care. Our therapists and rehabilitation specialists are committed to providing outstanding care to our patients. We rely on an interdisciplinary team of physical, occupational, and speech therapists to get you back on your feet as soon as possible.
