    This is Erie Counties only designated transitional care facility. Most patients have a length of stay of 4 weeks or less before returning home or to a lesser level of care. Located at the South Campus of Firelands Hospital most patients enjoy the ammenities of a private room with ensuite bathroom. This unit is often referred to as the bridge between hospital and home with many needing just a few days of additional care to enable a smooth transition to home. Longer term stays are welcome and the facility accepts patients from home and hospital settings. This facility is able to accomodate many private and managed care insurances along with medicare, medicaid and private pay. Wound care, infusion therapy, along with physical, occupational, and speech therapy are specialty services provided at this skilled facility. Admissions available 7 days per week in a hospital-based setting. Excellent patient to staff ratio. Tours available anytime.

    This facility is a transitional care unit with most patients goal to return home or a lesser level of care. With advanced Alzheimer's, patients are referred to an appropriate long term facility with a specialized care unit designated for this purpose.
    Special care programs offered for Alzheimer's and Dementia residents:
    No specific programs available at this facility. Care of patient is based on their specific needs and level of cognition. Patients generally are shorter term with cognitive abilities which allow them to progress with therapy as appropriate and return to a lesser level of care. Facility also operates an in-patient hospice program with focus on comfort and quality of life.
