    Wauconda HealthCare & Rehabilitation Centre was nationally recognized for its Choice program, an acronym for Choosing How Our Individualized Care Evolves. Choice is a person-focused care model that responds to the desires and preferences of guests and their families, and has proven successful in significantly improving guest function and well-being. Wauconda HealthCare was awarded the 2006 Optima Award for excellance in healthcare for its teamwork, strategies, and programs. Wauconda HealthCare & Rehabilitation Centre strives to provide a comfortable environment that feels like home. With a big screen T.V. and movie area, bistro with Internet café, library and two enclosed gardens, there is plenty of room to relax and enjoy the many activities available. Our guests also enjoy regularly scheduled outings in and around the community on our fifteen-passenger bus. Each member of the care team takes the time to offer friendship as well as their expertise. Wauconda HealthCare’s team consists of licensed and registered nurses, licensed therapists, and rehab-certified nursing assistants who work to assist the guests in obtaining and maintaining their highest level of function. The goal is to discharge seniors back to their homes whenever possible, providing excellent short-term care.

    Staff certifications and training:
    Each new staff member is mandated to paraticpate in our facility orientation, this is then followed by departmental education based on the Illinois Department of Public Health requirements. All staff our required to attend a 4 hour Alzheimer's training course and those staff members working in our Specail Care Unit are required to do an additional 12 hour course on Alzheimer's care.

    This facility can handle most medical conditions, however any behavior that diplays possible harm to another patient, themself or a staff member is grounds for discharge.
    Special care programs offered for Alzheimer's and Dementia residents:
    This facility offers a 31 bed unit with 3,400 sqft of activity space designated to those inviduals with dementia.

    Because this facility owns a 15 passenger bus, any community outing is available. Thanks to the wonderful support of our community, many services are provided by volunteers such as religious services, Bingo and a wide variety of other activities.
