    Avamere Rehabilitation of Beaverton is conveniently located off of Highway 217 on Allen Boulevard and situated on a beautifully landscaped campus in a cozy residential neighborhood. We’ve recently renovated our building to enhance the comfort and experience of our patients, residents and guests. Our caring team of nurses and rehabilitation therapists work closely with patients, family members and medical staff to provide a personalized treatment program for those recovering from illness or injury. They work together to help restore patients’ health and spirit on the road to recovery.

    Eligibility requirements:
    Physician’s order to SNF/ICF admit; prior auth by facility DNS and/or administrator (acceptance); confirmed payer source (HMO, MCR, MCD, PVT) if private pay, must have 30 days advance payment upon admit.
