    Sibley Nursing & Rehab Center is a 51-bed skilled nursing facility located at the north end of Sibley in northwest Iowa. Our Center is dually certified in Medicare and Medicaid. Our Center is strongly supported by numerous committed volunteers. Our activity program is lively and stimulating. It provides a progressive program seven days a week. Our facility gives our residents beautiful areas for dining and relaxing, including a patio for outdoor enjoyment. Delicious meals are served elegantly, with linens, place settings, goblets and flowers. Open breakfast is offered seven days a week as well as meal choices for the noon and evening meal. A fabulous dessert cart is offered weekly. Therapeutic and rehabilitative services are available through our Occupational, Physical and Speech therapists. Together they provide encouragement and skilled services. At our nursing Center, the commitment is to provide the highest quality of service in an environment filled with love and dedication. We want you to know that we care about you and your family. We hope that you will come and see us. Sibley is just two miles from Hawkeye Point, the highest elevation in Iowa. Sibley boasts cobblestone streets, nostalgic lampposts and a main street with country charm filled with interesting shops.     Staff certifications and training:
    All certified nursing assistants have completed the 75 hour training course and are certified in the state of Iowa. We have Registered Nurse coverage at a minimum of 8 hours a day per state requirements. We have Licensed Practical Nurses on staff at a minimum of 16 hours per day. We have nursing staff available 24 hours per day. Certified Nurse Aides are provided 12 hours of continuing education each year. In addition all nurses licenses are up to date and all nurses have met their continuing education requirements. Retraining is offered to any staff member in any department at any time. We want to make sure our staff are comfortable and confident in their job duty.

    Diminished abilities or stage of disease that would result in a transfer or discharge:
    This facility does not have a secure dementia unit within it. That does not mean that someone with dementia or Alzheimer's disease is turned away from placement. The facility will discharge a resident if they become a harm to themself, other residents, or wanders outside of the facility and is an elopement risk. Residents with dementia or Alzheimer's disease live with residents who are not afflicated with memory loss. Safety for all residents is the number one concern. Residents with a high potential for wandering will have a wander guard in place. That is a small bracelet that will alarm if the resident goes through an exit door.
    Special care programs offered for Alzheimer's and Dementia residents:
    Care plans are made to tailor specific needs of a resident. Every resident has their own daily routine. One on one activities are available for residents with lower cognition levels. As a facility we try to meet all of the resident's unique needs.
