    Highlights
    Our Center is a person-centered environment constructed in the Neighborhood model. We have twice been featured in national long term care magazines for our design and resident empowerment efforts. We offer a state-of-the art environment for residents to enjoy enhanced quality of life and quality of care. We have pets, plants and primary care staffing that makes a resident's stay purposeful and engaging. Resident's self-determine the hour they wake and go to bed, the manner and frequency with which they bathe and dine in country-style dining rooms at the hour they choose. We have dedicated neighborhoods for transitional care, memory care and bariatric care. We offer hospice, respite, outpatient rehabilitative services and adult day care programming. Our employees are dedicated to working together across shifts and across departments to achieve the highest level of resident satisfaction and participation in their care. The Center maintains active Resident, Family and Employee Councils to guide the organization.

    Special care programs offered for Alzheimer's and Dementia residents:
    We offer a 48 bed Memory Care program called Memory Lane. Program Staff and environmental management plan developed in conjunction with the Alzheimer's Association. We have a 12-bed Bariatric Care Program for individuals with body weights not exceeding 650 lbs.
