    Dedicated to providing the highest quality services within an atmosphere of comfort and hospitality for more than thirty years, Grand Rapids Care Center is proud to offer a complete continuum of services. The Grand Rapids Care Center offers skilled, intermediate, and various specialty long term care programs. Conveniently located in historic downtown Grand Rapids, the facility is easily accessible from Toledo, Bowling Green and adjacent communities. Specializing in comprehensive rehabilitation, including on-site physical therapy, occupational therapy, speech therapy and restorative nursing. The nursing staff is well respected throughout the medical community for providing personalized care to individuals who have skilled or complex needs such as wound care, diabetic monitoring and teaching, post-surgical treatments and pain management. Long-term care services are also offered, including hospice care. The accommodations are exceptional, including spacious shared arrangements. Grand Rapids Care Center is well known for its home style cooking. All meals are freshly prepared from our kitchen and served with smiles. Our Activities programing includes; Outings in our bus, parties, games, socials, art and exercises.. . these are just a few of the activities we plan each day. The short-stay ("respite”) program allows individuals to experience being part of the Grand Rapids Care Center family of services for a long weekend, several weeks or even a month at a time. It allows caregivers to take breaks with confidence. Admissions are coordinated on a 24-hour/7-day a week basis for your convenience. Private meetings and tours are gladly scheduled to meet today's busy schedules; including evenings and weekends. Grand Rapids Care Center is an approved provider under the Medicare and Medicaid programs. Most private insurance plans are accepted.
