    Willow creek adult family home is a community-based living option for adults with severe and persistent mental illness and developmental disabilities. Also clients with Alzheimer’s disease in many different stages of the disease. Care and services are provided to meet individual needs and promote care in the manner and environment that maintains dignity and respect. Willow creek is a four bed; state license AFH that promotes wellness and recovery in a homelike setting. The home is located in a residential neighborhood at 1465 Avondale drive. Ashwaubenon, WI 54313 a city transit stop is located two blocks from the home, which provides convenient access to the city via green bay metro transit. The home has two socializing areas for comfortable gatherings. A spacious kitchen allows for teaching meal planning, preparation and family style dining. All residents enjoy the comforts of a private bedroom. Furnishings can be provided by home or you may bring your own. The home has four bedrooms and two large bathrooms. There is a large wheel chair shower. The home is wheel chair accessible, equipped with two exit ramps. The home is a non smoking home, except in designated areas. There is a large deck around a heated swimming pool and hot tub, equipped with special lift for easy and safe access to both.
