    About My Business
    Trinity Senior Community is a memory care community designed and specifically operated to meet the challenges of Alzheimer's and other dementia in a residential setting. We are dedicated to providing the highest quality and individualized services to our residents. We promote and encourage autonomy, decision making, and provide socializations and activities. Our personal care and supportive services meet the needs of older adults who have some limitations due to their advanced age or memory impairment. We are an alarmed entry home with an enclosed courtyard and are fully handicapped accessible.

    • 24 hour supervision and interaction with caring staff specially trained in the care of Alzheimer's and related dementia

    • Home-cooked meals served in our main dining room; snacks available everyday
    • Individualized service plans designed to meet our resident's special needs

    Conditions I Am Unable To Treat
    • Unable to care for patients who require tube feeding
    • confined to bed by illness or infirmities, destructive of property, self or others physically or mentally, needs 24 hour supervision by RN or LPN, or is medically unstable, UNLESS hospice, psychologist, or mental health agency is involved.

    Upon admission each resident undergoes a thorough assessment concentrating in the following key areas. Based on this assessment daily service points are assessed and basic levels or care fees are figured:

    • Dressing/Grooming
    • Communications, includeing speech,communication tools, and family participation
    • physical abilities including mobility and stability
    • restorative program
    • personal hygiene/continence
    • bathing/showering
    • health/wellness, including skin care, medications and diabetic management
    • dining services
    • envrionment, including envrionmental assistance, housekeeping,laundry assistance
    • habits/routines including tobacco and alcohol use
    • cognitive status including orientation, decision making
    • psychological well-being, including wandering and specific behaviors
