    Diminished abilities or stage of disease that would result in a transfer or discharge:
    Heritage Homes admits up to Stage 5 Alzheimer's patients. However, a patient who has been already been admitted to our campus will not be discharged based soley upon progression to later stages of Alzheimer's. It is our goal to have each individual age in place with dignity and grace.
