    "My Home" is a spacious (3,000 square ft) one floor living, beautiful home located in a picturesque and secluded neighborhood. We are approximately 3 miles north of I-69. We are just blocks from Historical downtown Lapeer, Lapeer Regional Hospital,,churches, and shopping. We are a 'one stop shop'. Because I am an RN (with 5 years Hospice experience) there would be no need to seek alternate living arrangements as most needs can be provided. Provisions include but are not limited to the following: TRAINED PERSONNEL Professional, quality and compassionate care is provided 24 hours a day, 7 days a week. SUPERVISION AND PERSONAL SAFETY • I reside in this home and am the primary supervisor for care. In the event of an emergency, arrangements are in place for qualified coverage (RN or LPN) up to 72 hours. • Daytime care-givers are Certified Nurse Assistants. • I will employ only those who acknowledge and advocate my resolve to provide quality care. • All employees are required to complete related care giving in-services or workshops throughout the year. • Employee and caregiver criminal and fingerprint background investigations. • Annual fire alarm inspections and drills • Annual home (structural) inspections • Home security system PERSONAL ASSISTANCE • Bathing, Grooming and Range of Motion • Hair Care Shampoo, conditioning and daily styling (rollers, curling iron, flat iron, blow dry styling). • In-Home Salon services once a month • Nail Trimming RN ONLY • Basic Manicure and Pedicures • Laundry Machine Wash/Dry • Exercise Program A personal trainer will work with the staff to determine and implement an exercise program designed for you. • Massage Therapy Once a month ACCOMODATIONS • 2 private bedrooms 12x12 and 2 private bedrooms 12x13. Private rooms are provided with a full size bed, dresser or chest of drawers, comforter, bed and bath linens, a call system and room (voice) monitor. • 1 Semi-private bedroom 14x24. Has a walk-in closet, attached bath, single beds, dresser, comforter, bed and bath linens a call system and room (voice) monitor. You are at liberty to decorate your room (room décor, wallpaper, paint etc.) to your individual taste. UTILITIES Electric, gas, water, sewer, existing cable and telephone package are included. Meals • 3 meals a day, served at the family table, each planned under the guidance of a nutritionist. • Prescribed dietary needs will be accommodated. MEDICAL ASSIST NEEDS • Medication Administration and Treatments as prescribed by your physician. • Each Health Related Service (examples include catheter, ostomy care, gastric tubes and so on) will be taken into consideration and collaborated with AFC consult prior to acceptance. Acceptance of health related service will be based on RN’s spectrum of licensure and area of experience. RECREATIONAL AND LEISURE ACTIVITIES • “My Home” sponsored teas, picnics, gatherings and events. We will assist you in hosting occasional family, social or community events. • Family visit, shopping or library day • Flower/vegetable gardening • Holiday planning, preparation and decorating • Planned daily activities • Assist in personal activities such as writing letters, computer use, reading, etc.

    Myself and both CNA's are hospice trained. Therefore unless a patient becomes a danger to others, there would be no need to discharge.
    Special care programs offered for Alzheimer's and Dementia residents:
    I have none established at this time outside of what Home Care offers. However, I would integrate any services that are available to me to meet my patients needs.
