    Our Skilled Nursing Unit is state-of-the-art with large private and semi private rooms available. Our facility sits on 41 acreas of natural woods and wetlands, yet is within minutes of transportation and shopping. All staff at Clare Oaks is required to attend an minimum of 4 hours in dementia training yearly and our nursing staff receives more intensive training. We are equiped to successfully treat almost every medical disease process and not only offer outstanding physical care to short and long term residents, we strive every day to live our motto: "A Community For Mind, Body & Spirit".

    In our skilled care area, the ratio of resident/nurse is 15 to 1. For CNA's the ratio is 6 to 1. All mandatory requirements for continuing education is provided to all staff. Staff is specially trained in dementia care.

    Clare Oaks is a premier facility that offers independent living, assisted living and skilled nursing care. We have a convenient location in Bartlett Il that is close to transportation, shopping and leisure activities, yet we sit on 41 lush acres of woodland. There is always something to keep you busy at Clare Oaks, but if just relaxing in our indoor pool, meeting friend in the lounge or having a leisurely meal in our beautiful dining room is more your style, we can certainly accommodate that! In our Assisi Healthcare Center, we offer Assisted Living, a specialized Memory Support Unit and dedicated skilled nursing. Our licensed staff-to-resident ratio is very low, so that you or your loved one can experience the most efficient and up-to-date care available. And we will even have time to sit and visit with you!
