    The story of the legend of the phoenix bird is a story of ever-changing growth and rebirth. The beautiful mythical creature reflected the lights and colors of the morning sunrise – – always chasing the light and capturing all that was beautiful.     About My Services
    Phoenix welcomes the opportunity to provide skilled services to persons meeting Medicare Home Health Coverage Criteria:

    The person requires intermittent skilled care provided by a nurse; physical therapist, speech or occupational therapist; is home-bound, meaning the person has an illness, medical condition or injury that restricts the ability to leave home without the assistance of others; the aid of supportive devices such as crutches, canes, wheelchairs or walkers; the use of special transportation; or those instances where leaving the home is medically contraindicated.

    Once the patient has met the coverage criteria, a physician must then order services. Our staff and clinicians will work directly with that physician and staff to customize a plan of care to the specific needs of that patient.

    Conditions that generally meet the coverage criteria include:

    A new diagnosis
    New type of medication or medication dosage change
    A recent fall
    Recent exacerbation of a previous diagnosis
    To qualify for Medicare or Medicaid Home Health Services, your home health agency must be Medicare and/or Medicaid certified. Phoenix Home Care meets this requirement. Home health care services may be covered under Medicare Part A or Part B.

    Skilled Nursing Services

    Skilled observation and assessment of a newly diagnosed condition or the exacerbation of an existing condition

    Teach nasogastric feedings
    Re-insertion nasogastric feeding tube
    Teach gastrostomy feedings
    Teach parenteral nutrition
    Teach tracheostomy care
    Teaching in administration of medications, including inhalants and oxygen therapy
    Teach administration of injection
    Disimpaction/FU enema
    Management and evaluation of a patient care plan
    Change in Medications within the last 60 days
    New Medications in the last 30 days
    Change in Primary Caregiver providing care with a knowledge deficiency
    Skilled Nursing related to a treatment of an illness or injury that must be provided by a nurse

    Privately Paid Services
    Privately Paid Services and Private Duty Nursing/Adults provides activities of daily living within an individual’s home. These services are designed to eliminate the need for unnecessary institutionalization by creating a safe living environment, fostering independence and facilitating rehabilitation. Services are available to youth and adults who make arrangements for individualized care and private payment for services.

    Privately Paid Services are available for 8, 12 or 24-hour shifts. Hourly services are also available for those needing less than an 8-hour shift. Just a few examples within this service offering:
