    ElderCare Solutions of Michigan is a Geriatric Care Management division of Jewish Family Service. We help older adults with all need areas they may have. Our services are designed to

    An ElderCare Solutions of Michigan Care Manager Can:
    • Conduct a professional assessment
    • Create a client-centered care plan
    • Arrange age-in-place care services
    • Provide crisis intervention
    • Be there to lean on for support and guidance
    • Advocate at medical appointments
    • Assist with residential planning
    • Ensure continuity of care from hospital to home
    • Help family members cope with the physical and emotional issues of aging
    Experience And Credentials
    Our Care Managers come to you with over 50 years of combined experience in Geriatric Care Management. They are compassionate, caring, loving people who truly look out for the best interests of our clients. Our team includes professionals from varying backgrounds with degrees in the following areas:
    • Masters in Social Work
    • Masters in Gerontology
    • Registered Nursing
    • Masters in Health Administration

    We are also members of the National Association of Professional Geriatric Care Managers
