    About My Business
    Adult Day Service Programs provide seniors and disabled adults opportunities for exercise, socialization, and cognitive therapy while their family caregivers receive the benefit of respite. These services help prevent physical and cognitive decline of its participants as well as caregiver burnout, thus delaying and/or eliminating living facility/nursing home placement. Grace Center has been promoting health and dignity through our adult day services since 1983 and remains the only adult day program in the Corvallis and Linn-Benton County region. We hold specialties in Memory Care and Post-Rehabilitation.
    About My Services
    Program hours are 8:00 a.m. to 4:30 p.m. (extended hours available for additional fee). Participants may attend hourly up to full days on a set schedule or as needed.

    Our program includes

    •An individualized plan of care based on the functional abililities and goals of each participant
    •Health monitoring and consultation with a registered nurse
    •Medication administration and coordination with the participant’s primary care provider
    •Social services and support for the participant and caregiver
    •Nutritious lunch and morning beverages


    •Physical, occupational and speech therapy follow-up
    •Gym with specialized fitness and exercise equipment
    •Modified group exercise classes
    •Walking program and specialized mobility equipment
    •Strength and endurance training
    •Range of motion and stretching exercises
    •Mentally-stimulating games and puzzles
    •Art and craft classes
    •Outdoor raised-bed gardening
    •Discussion groups and presentations
    •Musical entertainment
    •Parties and field trips

    Memory Care

    •Safe and secure indoor and outdoor environments
    •Outdoor therapeutic garden with raised beds
    •Indoor park with games such as golf, basketball, horseshoes
    •Group exercises for flexibility and strengthening

    •Mentally-stimulating games and puzzles

    What does it cost to attend Grace Center?

    •Full day fee = $92
    •Half day fee = $64
    •Hourly fee = $22/hr.

    Long term health insurance policies usually pay for adult day services, and Veterans Services may be able to help. We also have a contract with the State for Medicaid clients, Benton County Developmentally Disabled and Oregon Project Independence may also be available. We have always been committed to providing discounts and accepting reduced payments from government based programs so that individuals from all income levels can benefit from our services.

    8:00 am to 4:30 pm (up to 8.5 hours) = $92.00

    Any four hour block of time = $64.00
    (if present from 11:45 am to 12:15 pm, lunch is included)

    •Daily exercise programs (group classes and supervised ambulation)

    •Activity supplies and craft materials
    •Blood pressure, pulse, and weight monitoring
    •Individualized plan of care
    •Personal care services, if needed
    •Health care consultation and monitoring by Registered Nurse
    •Medication administration and monitoring, if needed
    •Lending library
    •Noon meal and beverages (if present from 11:45am to 12:15 pm)

    Other Amenities

    •Hourly Rate = $22/hr. (includes lunch if present from 11:45 am to 12:15 pm)
    •Extended Hours = $11/30 mins. (must be scheduled in advance)
    •Shower = $24 (supplies provided)
    •Massage Therapy = $1/min. (15, 30, 60 minute increments)
    •Other (please ask us about other amenities that may be available)
