    Adult Day Health Care services to clients meeting Level I nursing home criteria.

    Located within a Federally Qualified Health Center that supplements services with primary care, medical social services, skilled nursing, and rehabilitation services.
    About My Services
    Serve cleints needing assistance with ADLs, restorative services, medication,feeding, recreation,activities, meals, baths, and alzheimers.
    Conditions I Am Unable To Treat
    Cannot serve more than 16 persons per day that are confined to Geri-Chairs for safety reasons.
    Experience And Credentials
    Administrator is licensed nursing home administrator with 22 years active license and 34 years health care administration and services. Administrator holds AS, BS, MHA, and PHD in health services and administration.

    Program Director is a Medical Social Worker with four years long term care experience. Currently BSW obtaining MSW May 2010.
    Fees And Payment Sources Accepted
    Our base fee is $10 per hour. We charge $12 per hour for most individuals that are diagnosed with alzheimers.

    We accept HCBS Waiver, insurance, and use a sliding scale for uninsured persons with incomes less than $27,077 per year.

    8:00pm to 4:00pm. Extended hours during none program hours are extra. Extended hours can be arranged for times between 6:00am and 6:00pm.
