    About My Business
    As a Geriatric Care Manager and a Licensed Clinical Social Worker, I help families who are caring for older relatives at a life changing time.
    A Geriatric Care Manager assists older adults and persons with disabilities in attaining their maximum functional potential. In addition, the Geriatric Care Manager is an experienced guide and resource for families of older adults and others with chronic needs, including helping those suffering from Alzheimer’s Disease or Parkinsons or exhibiting symptoms of dementia.
    The Geriatric Care Manager meets the senior and his family members where they are psychologically,emotionally , physically, and spiritually. The Geriatric Care Manager has the knowledge, support, and experience with seniors and families to assist assist the senior and family through a life cycle event.
    About My Services
    Care management: identifies problems, needs, and strengths that may inhibit or enhance a seniors living arrangement
    Deveelop an indiviualized plan to resolve problems and meet those needs
    Implement the plan by arranging for the necessary serives
    Assume responsiblity for coordinating and monitoring the delivery and quality of care
    Report to the family changes in seniors conditon and needs
    Counseling to assist the senior and family with the changes they are facing
    Conditions I Am Unable To Treat
    Unable to service:
    psychotic people
    people who are mentally ill and are unwilling to allow psychiatric intervention
    physically abusive or threatening people
    waepons in the home that have ammunition available
    Experience And Credentials
    Fees And Payment Sources Accepted
    Home visit 1st hour $200.00
    Each hour after the initial is prorated in 15 minute increments at $37.50
    Phone and resource aquisition is prorated in 15 minute increments at $37.50

    8:30 A.M. to 8:00 P.M. M-Fri
    Saturday and Sunday by appointment
    I am unable to do night interventions
