    Overview of Services

    Here is a general list of the array of services that Denise Robinson, LCSW, CASWCM will offer the senior and his/her family:

    Develop a customized home assessment to identify problems, determine assistance eligibility, and make appropriate community referrals.
    Respond with crisis intervention in the form of counseling and devising an action plan to prevent further similar situations.
    Conduct a thorough review of financial, legal and medical issues, and make referrals to specialists to conserve assets and avoid future problems.
    Assist with short- and long-term planning and coordinating services.
    Conduct mini-mental exam, depression assessment, and make referrals for psychiatric evaluations.
    Act as a liaison with family members to ensure safety and to alert them to potential problems.
    Assist with relocation and related issues if it becomes necessary to move.
    Coordinate with hospital discharges and planning for home needs.
    * Maintain an ongoing assessment of needs and progress and adjust services as needed.

    Healthcare Services
    We will manage all medical and dental appointments. We assist with health insurance issues, prescription maintenance, nutrition and meal services, and any medical equipment needs.

    Personal Services
    We will assist with home and companion care, transportation needs, financial management, errands, housekeeping, bill-paying, travel, long-term care insurance or daily correspondence.

    Relocation Services
    We will assist with recommendations and coordinate services for the senior to move to the appropriate level of care, depending on care needs.

    Legal and Financial Services
    We offer direct assistance with advanced directives, durable power of attorney, trusts and living wills, MediCal, long-term care insurance issues, and conservator referrals.
    About My Services
    Our Steps

    We visit your senior family member, wherever he or she is - at home, in the hospital, in an assisted living or nursing home. We conduct a complete and thorough evaluation of the needs in the following areas: physical, mental, emotional, financial and environmental. We then review these needs with the involved family and with the senior client to develop specific goals and recommendations.

    Following the goals from the Initial Assessment, we coordinate with the senior and any family members involved to create the Individualized Plan of Care. All parties have the power and control over the services that are received. We offer recommendations, options, resources and assistance to coordinate the services. The final decision rests on the combined efforts of the team of client, family and us — your case managers.

    We remain involved in the senior’s life on a regular basis. We make frequent phone calls to ensure the safe plan remains in place. We are available by phone and always return calls within 24 hours — usually sooner. We provide supervision, scheduling and advocacy at all times. We continue to assist the goal setting as needs change or improve. We stay in touch with you, the family member, as often as you wish.

    Private pay. In many instances, insurance companies will reimburse the client for a portion of the services fee.
