    About My Business
    Care Solutions for Elders is located in Marin County with an emphasis of Medical Case Management, Advocacy, Transitional care, Dementia Care, and Consultation for elders and their families. Our team consists of Registered Nurses with over 25 years in the field of gerontological nursing. We interface with Medical Disciplines and are able to provide the assurance families need when faced with care, challenges, and changes that occur in aging. We work collaboratively with families and elders as well as with other health care professionals to ensure the least restrictive environment, aging in place, along with providing a plan of care, and guidiance. Our goal is to provide information that will allow each elder and family to make formative decisions. We also provide professional documentation to professionals as well to families outlining a plan of care, recommendations, and resources.
