    About My Business
    SeniorCare Family Services, Inc. is a Geriatric Care Management company that assists seniors In navigating the health care system. Our expertise is in managing both complex medical needs and the problems of every day aging. We advocate for our clients, and bring peace of mind to families not able to be active in their loved ones day to day life. If living in a facility makes sense, we can assist in finding the right placement that fits both financial and level of care needed. Our services encompass monitoring care in both facilities and in home providers. SeniorCare Family acts as a safety net for the senior and is on call for their needs twenty four hours a day. We work to keep our clients safe, independent, and enjoying life while keeping families informed. Our providers are all registered nurses with extensive backgrounds in geriatric care.
    About My Services
    Assessment Only
    We offer an assessment only option for those families or guardians who would like a comprehensive document outlining the seniors current status of function. A care plan is included that lists problem areas with suggested solutions. The care plan acts to educate those caring for the senior on his/her strengths and weaknesses and helps to guide planning for quality of life.

    Phone Consultation
    Sometimes families find they need education about local resources or help in making care decisions. They may lack the resources to engage a care manager for ongoing services. We provide telephone consult billed on an hourly rate for those on a limited budgets needing professional guidance. While this cannot replace ongoing care management it can provide needed information for families struggling to may informed decisions.

    Facility Placement
    When the decision is made that an elder needs to be in a "community of care setting" we can assist the family in making the right match. Considerations need to be given to level of care needed, but also to the types of activities offered, is there a nurse on duty round the clock, is the staff stable, is the facility in an acceptable location and does the cost fit the client's budget? We can offer a one time consultation or an hourly rate for taking the family through every step of the decision process.

    Ongoing Monitoring
    The on going relationship between the care manager and the client starts with an assessment of needs and a care plan is developed to resolve identified problems. The care manager is in contact with the family to give updated reports and a schedule of visits is agreed on with the family and client. The care manager typically attends all doctor visits, monitors the in home services, and is on call for the client 24/7.
