    Assess the Client's particular needs and expectations, assist the Client/family in finding the most appropriate care-giving options

    Represent the Client, in all matters, from a position of professional competence and experience.

    Assist families in administering their decisions - an unbiased professional can often times act as an intermediary in making critical life issues i.e. Living Arrangements (Managed In-Home Care, Assisted Living Facility or Skilled Nursing Home).

    Help the Client adjust to their new environment and life-style.

    Be an advocate for the Client
    intercede when the care-giving responsibilities cause tension among family members.
    About My Services
    Geriatric Care Management includes:
    1. Assessment and evaluation of current and anticipated needs
    Advocacy
    2. Arrange and/or monitor home services
    3. Coordinate other care providers
    Crisis intervention
    4. Identify needs
    5. Liaison for long distance families
    6. Post Hospital Discharge planning
    7. Referrals to community service agencies and geriatric specialists
    8. Research alternative living arrangements and assist with moving

    9. Review medical and safety issues

    10. Telephone conferences/consulting

    Fees And Payment Sources Accepted
    1. $350.00 for an initial assessment for a client living at home (1)
    2. $250.00 for an initial assessment for a client living in an Assisted

    Living Facility (ALF), or a Skilled Nursing Facility (SNF) (2)
    1. $95.00 hourly rate, thereafter. This will be for time dedicated in

    researching caregiving options and presentations to the contracting party (you), and/or follow-up visits or time dedicated to the client, as requested by the contracting party (you).

    (1) Our initial interview with a new client living in their own home includes: testing for cognitive function (Folstein Mini-mental status examination, "FROMAJE", "TYM" or "Boston Naming Test" Mental Status Evaluation, Emotional State (Mood Scale Test), Functional Status, an Assessment of Personal Hygiene and a "general safety inspection" of the residence. Written report to contracting party.

    (2) Our initial interview with a new client in a Nursing Home or Assisted Living Facility (ALF) includes: testing for cognitive function (Folstein Mini-mental status examination, "FROMAJE", "TYM" or "Boston Naming Test" Mental Status Evaluation, Emotional State (Mood Scale Test), Functional Status, interview with hands-on caregivers and social workers, etc. Written report to contracting party.

    Note: As Professional Geriatric Care Managers, we do not contract directly with the Client, but rather contract with a responsible party, such as yourself. This assures that a neutral party (not the Client), determines the number of hours that we dedicate to the Client. Most time however, the payment for our services comes from the assets of the Client, not from you, the contracting party. Our Fees are usually paid by the person appointed by the Client as their Power of Attorney (POA) from the assets of the Client.
