    About My Business
    As a Professional Care Manager, I assist individuals with all aspects of their lives. My goal is to assist older adults and their families, meet the challenges of aging, by focusing on capabilities and desires rather than limitations. I collaborate with a network of other experts to facilitate the best quality of life for my clients.

    My services are geared towards serving the needs of seniors and their families, but I also provide case management services for those with disabilities, chronic diseases (like diabetes, COPD, heart disease and arthritis) and catastrophic diagnoses (such as cancer, stroke, brain and/or spinal cord injuries).
    About My Services
    My services begin with a telephone consultation, followed by a visit to the care recipient and their family, to complete a comprehensive assessment of medical, social, financial, and legal concerns.

    Following the comprehensive assessment, problems and goals are identified and a care plan is developed. With client approval, recommended services are coordinated and ongoing care needs are monitored.

    The total cost of Case Management services will depend on the frequency and intensity of intervention that is requested. Sometimes just a little assistance can allow a loved to remain in their home but if other living arrangements are needed, I can assist with alternative arrangements, such as selection of, and transition to, an assisted living facility (ALF). I am knowledgeable of a wide variety of community resources and support options that can help make life easier.

    Care Managers save money and reduce family burden by coordinating appropriate care and resources and avoiding unnecessary expenditures. One of my many goals is avoidance of crisis, unnecessary hospitalizations and inappropriate placements. Care Managers can also reduce costs by negotiating discounts for medical services and supplies.

    I have been a Registered Nurse in the Tampa Bay area for over 30 years. I was one of the first professionals to obtain National Certification as a Case Manager (CCM) in 1993. In 2000 I also obtained National Certification as a Rehabilitation Registered Nurse (CRRN). I completed the Florida Assisted Living Administrators (CORE) training program, followed by certification through The Florida Department of Elder Affairs in 2006.

    I am an active member of the National Association of Professional Geriatric Care Managers (NAPGCM), the Florida Geriatric Care Managers Association (FGCMA) and the American Holistic Nurses Association (AHNA).
    Fees And Payment Sources Accepted
    Care Management is a private pay service. Fees are not generally covered by Medicare, Medicaid, or most health insurance policies. Occasionally fees are covered under a Long-Term Care Insurance Policy, and we will be happy to assist you in determining if your LTC plan would reimburse you for all or part of our services.

    The total cost of Care Management services will depend on the frequency and intensity of intervention that is requested by the client. Professional Care Management involves and in-depth assessment, the development of an individualized plan of care, service arrangement with on-going follow-up and monitoring. While we believe this is the most effective way to serve our clients' needs and enhance their quality of life, we also realize that budget restraints may not permit them to take advantage of the full service we provide.

    In addition to our standard care management services, we also provide professional consultations, by phone or in person, at an hourly rate. This in no way replaces the benefits that can be derived from full Professional Care Management services, but it will permit those on a limited budget the opportunity to learn about various options, services, and community resources.

    We strongly believe that information is power and that the valuable information we will provide during a one-hour telephone consultation will permit you to make better choices and more informed decisions regarding specific problems or concerns.

    As Care Managers we believe our services will save you time, money, and reduce family burden through coordination of appropriate care and resources and avoidance of unnecessary expenditures. One of our many goals is avoidance of crisis, unnecessary hospitalizations, and inappropriate placements.

    In addition to accepting payment by cash or personal check, we are pleased to announce we are able to accept payment via VISA, MasterCard, and Discover card.

    Most of our business takes place during "normal" business hours, Monday-Friday from 8 AM-5 PM.

    We can arrange to meet with those who require conferences or assessments after hours or on weekends on a case by case basis.

    Our services are available 24/7 for those who are are receiving on-going Care Management Services.
