    Elder care planning and coordination can be overwhelming. When the decisions are difficult and the answers are hard to find, our Aging Life Care™ managers can help.


    We work with individuals and families with the goal of optimizing the quality of life and well being of the older adult. We conduct in-depth assessments, develop care plans and recommendations, coordinate services, act as liaisons to health care providers and insurers, and continuously monitor services to ensure that the individual’s needs and goals are met.

    Care managers are experts in working with individuals to identify their goals and locate the specific support services that enhance well-being. When faced with the array of choices and challenging decisions, care managers provide support to find the best solutions and help make the caregiving process as positive as possible.

    Assessment and Care Plan

    We are ready to listen and help you problem solve. We can help you find creative solutions to your most challenging problems. We provide a comprehensive assessment to review the medical/mental health issues, cognitive functioning, functional status, support systems, legal, financial, home safety and nutritional status. Once we obtain a clear picture of the older adults current situation and health care needs we take a collaborative approach with the client and their support system to develop a customized plan of care. We can implement the care plan and provide ongoing monitoring and support of the plan.

    Community Resource Information and Referrals

    ElderSource can provide trusted and reliable referrals for medical specialists, home care agencies, elder law attorneys, financial planners and other professionals. We have extensive knowledge about the cost, quality and availability of services in the Charlotte, North Carolina area.

    Care Coordination and Oversight

    We can help screen, arrange and monitor in-home care and other services. Services may include caregiver or companion support, transportation, socialization, meal preparation, bill paying services, financial management and assistance with medication management. Ongoing safety checks are done to review changing needs of the client.

    Accompany client to medical appointments

    Care Managers can accompany your loved one to medical appointments, tests, outpatient procedures. We help with the facilitation of communication between all involved health care providers, client understanding of adherence to physician prescribed medications and treatments and comprehensive reports to family members.

    Crisis Assistance and Intervention 24/7

    We provide 24/7 on call service for our clients when there are unexpected changes requiring urgent decisions or immediate attention such as emergency room visits. This provides peace of mind to our clients and their family members living at a distance.

    Relocation Assistance

    We can research, evaluate and arrange for senior living options such as: assisted living communities, long term care facilities, and dementia specific residences. We can coordinate all the various aspects of moving.

    Patient Advocacy

    We are there to improve quality of life and to make sure the elder receives the best care possible at home, in a residential care facility or at the hospital. Our clients receive personalized health care advocacy whenever and wherever they We have the best interest of our clients in mind at all times, and we will advocate for excellent treatment from health care professionals and institutions. We support and promote the interests and welfare of our clients by taking the time to understand their needs wishes and goals. We can be a client’s voice when their health care status or cognitive status impedes their ability to advocate for themselves.
