    About My Business
I provide geriatric assessments to seniors residing in Central New Jersey. On completion of the assessment I develop a "plan of care" which I provide to family members with recommendations for the most efficient and affordable services to meet their loved ones care needs. I provide ongoing monitoring to clients and update the care plan as needs change and new challenges arise. In addition I make referrals to home care agencies, help with placement in adult day programs, locate and assist in placement in residential settings. I also provide families with referrals to medical, legal, financial and mental health provides as well. I enjoy helping log distance family caregivers by serving as their local support system and representative to agencies and facilities.
Conditions I Am Unable To Treat
I am licensed in New Jersey as an LSW. I am not a clinician and as a result I don't qualify for Medicare reimbursement.
