    Sole proprietor of Geriatric Care Management agency which serves older adults and families in Rhode Island. Services inlcude in-home assessments with full report of findings and recommendations with referrals. Advocate for older adults within the medical system through visits with doctors, intervening in the hospital, rehab and nursing home settings. Assist families with short and long term planning and placement issues.
    Experience And Credentials
    BSN, MPH, RN

    Bachelor's in Nursing from St. Anselm College
    Master's in Public Health from Boston University with a concentration in health services and geriatrics.
    Completed certificate program from the University of Rhode Island Geriatric Education Program. Certificate is in the "interdisciplinary Care of Geriatric Patients"
    Recently certified by the National Association of Certified Care Managers.

    Past experience includes positions in home care, adult day health services, geriatric psychiatry, program start -up and pharmaceuticals with medication management and compliance.

    Private pay with a flat initial assessment fee with an hourly rate after.
