    Range of services from one-time in-home assessment to on-going comprehensive care management for seniors and those with chronic conditions. Specializing in working with those with complicated medical conditions and/or dementia.
    About My Services
    I provide assistance with transitional care for seniors and others with chronic health conditions. Specific services available include:
    Comprehensive assessment
    On-going monitoring
    Medication assistance
    MD visits
    Coordination among health care providers
    Referrals to health providers and others
    Familiarity with services in the area, including those with no/low cost
    Recommendations for life-enhancing activities
    Communication among care providers and family members
    Family meetings/mediation
    Assistance with placement in various types of long-term care facilities
    Management of chronic health conditions, such as diabetes, arthritis, heart disease
    Arrangements made for in-home care, including on-going monitoring and intervention, as needed
    Conditions I Am Unable To Treat
    Some long-term care insurances will cover Care Management services.
    Experience And Credentials
    RN, MSN, CMC

    Many years experience in working with geriatric patients in a variety of settings, including clinical, educational, and research.
    Fees And Payment Sources Accepted
    Fees available upon request.
