    About My Business
    Froedtert and the Medical College of Wisconsin's Senior Health Program provides in-patient and out-patient services for medically complex older adults 65 years old and older. Our core team consists of a geriatrician, geriatric psychiatrist, nurse practitioner, advance practice nurses, social worker and psychometrist. Geriatric Care Management is provided through our home care provider Horizon Home Care and Hospice.

    Senior Health Care: Geriatric Primary Care and geriatric consultion.
    Mind,Memory,Mood: Geriatric dementia specialists and Geriatric Psychiatrists and their interdisciplinary teams work together to provide diagnositic evaluation of cognition, comprehensive assessment, expert medication management, access to clinical research, referrals for counseling, assistance for caregivers and hospitalizaiton when appropriate.
    ACE: Acute Care for the Eldering. Virtual in-patient service at Froedtert Hospital staffed by a geriatrician and geriatric nurse practitioner.
