    About My Business
    The basis of Continuum SCM is to coordinate and monitor services for seniors and disabled persons to insure that they receive care to effect the best quality of life physically possible and financially feasable.
    About My Services
    Continuum SCM's services start with a global needs assessment. The purpose of this assessment is to determine needs for Best Care and may indicate such service needs as: remaining at home with minor adjustments vs placement: safe discgarge from a hospital and possible need for follow up care monitoring after discharge for a limited period of time, or on-going: recomendation for a Durable Power of Attorney, bill paying service, entitlement program, Medi-Cal planning service; appropriate placement if remaining home is unsafe: and possibly we will determine a need that is so individual, it can not be forseen before assessment.
    Conditions I Am Unable To Treat
    Care Management is a consulting, monitoring, and Advocacy service. We do not diagnose, prescribe medications, write legal documents, or perform home repairs.

    We are mostly private pay, some Long Term Care Insurance will pay for Care Management, but not often.

    Hours of Service
    Office Hours are Monday through Friday 9:00 AM to 4:00 PM Pacific Time.

    A Care Manager is on call on weekends and Holidays.

    For on-going clients, there is a Care Manager assigned to take calls as need arises.
