    Lifecare Innovations is a Care Management company comprised of a group of dedicated professionals who deliver a myriad of Lifecare Services designed to handle just about any challenge an indificual may be presented with. Our professionals make the complex simple by untangling every aspect of an individual's life. Sine 1995, Lifecare Innovations has earned its reputation as the company that can handle any problem. Our singular expertise in creative navigation of the healthcare, government, and guardianship systems has made our name synonymous with hope when all hope seems lost.

    • Advocate for the best care in all settings including hospitals, assisted living, and long term care
    • Manage simple day to day needs such as groceries, outings, housekeeping and bill paying
    • Coordinate client's participation in major events such as vacations, weddings, and graduations
    • Negotiate bills with medical providers and other services
    • Manage paperwork from bill paying to insurance forms
    • Assist clients in obtaining reverse mortgages and other creative financing to cover their costs of care
    • File for government entitlements, Social Security Disability, Medicaid and Medicare
    Experience And Credentials
    Our professional staff are all masters' prepared in areas of Social Work, Nursing, Psychology and related fields. Most of our staff are also licensed in their professional field. Many of our staff are members of NAPGCM or National Association of Professional Geriatric Care Managers, NGA (National Guardian Association), NASW (National Association of Social Workers), BBB (Better Business Bureau)and many other local and National organizations. Our company is the 2008 winner of the BBB Torch Award for Ethical Business Practices. Our President, Shay Jacobson, RN, MA has been named One of Twenty Two Influential Women in Business in year 2005 as published in the Business Ledger. Ms. Jacobson is a prolific local and national speaker and has been quoted in Forbes Magazine.
    Fees And Payment Sources Accepted
    We are a private pay organization. We accept long term care insurance for homemaker/companion/private duty services. Occasionally these policies will cover a portion of our Lifecare Management costs as well. Please contact us to review our pricing structure.

    Hours of Service
    We provide 24 hour on call services of a Lifecare Manager as well as through our Home Care Agency. Our office hours are Monday through Friday 8:30am through 5pm.
