    Services include: Assessments; long and short term planning; Financial planning and asset menagement; Assessment of benefit eligibility and assistance with apllications; Oversight of private/Medicaid homecare services; Assistance with payroll management; Effective linkage to medical, legal, home care and housing services; Counseling; crisis intervention and advocacy.

    Expertise in medical and skilled nursing home issues. Working knowledge of Article 81 (Guardianship) and collaboration with Guardians; Collaboration with Elder law attorneys regarding Medicaid homecare applications and NYSARC supplemental needs trusts.

    We are not a homecare agency. We assist clients in identifying private or agency aides and will advise clients regarding initiation of payroll, and can provide coverage coordination and oversight of staff.
    Experience And Credentials
    Post Master's Certificate in Gerontology from the Hunter - Brookdale Center on Aging.
    Certified Geriatric Care Manager
    Member of the National Association of Professional Care Managers.
    Board Certified Diplomate through the American Board of Examiners in Clinical Social Work,
    Licensed Clinical Social Worker in NYS
    Member of the National Association of Social Workers.
    Member of the Health Care Social Work Association.
    Adjunct Professor at The Fordham University in Gerontology Certificate program.
    Member of Fordham Ravazzin Center on Aging.

    Routine availability M-F 9-5. 24 hour availability for emergency situations. Flexibility for scheduling evening and weekend meetings/home visits.
