    About My Business
    Geriatric Care Management at CareForce is delivered by a RN and MSW team. We offer comprehensive assessments of physical, psychosocial, cognitive, environmental, and community needs. A comprehensive plan of care is developed and we then can provide all or portions of the plan of care depending on the company. Assessments may be used by clients, their families, trust bankers, or guardians in planning for both short term and long term needs. Examples of ongoing support include: scheduling and going to doctor appointments, supervising caregivers, medication management, organizing social events, monitoring ongoing medical issues.

    Fees And Payment Sources Accepted
    Comprehensive Assessment-$100.00 per hour - Assessments plus documentation can take 6-10 hours per person depending on complexity of issues.
    Geriatric Care Management Oversite $100.00 per hour calculated in 6 minute intervals, charge for travel, phone, in person time, emails, and documentation;$150.00 per hour after 5pm and on weekend days.
    Long Term Care Insurance that pays for geriatric care management

    Office Hours are 8a-5p Monday through Friday, Answer phones 24 hours per day, can assist clients after hours or weekends, increase charges apply after hours and weekends.
