    AZA Care Management and Home Care specializes in serving the geriatric, disabled adult and mentally ill. We are one of the largest and most prestigious Case Management practices in Massachusetts.
    About My Services
    AZA Care Management is an integrated and interdisciplinary team that focuses on and addresses a variety of individual client issues. Our overall goal is to develop a Care Plan with our clients and their families that nurtures independence for as long as possible. Our client Assessments include physical, cognitive and social funtioning as well as family and home environment concerns. With this information we develop short and long-term Care Plans for approrroate support services, whether at home, moving in with a loved one, an alternative housing community or a skilled nursing or rehabilititation facility.

    We save our clients time and money by putting together the right plan in place the first time.
    Experience And Credentials
    AZA Care Managers specialize in helping clients with Alzheimer's disease, related dementias and other neurological disorders.

    AZA Care Managers have extensive experience with adults who have suffered disabling events such as head injury, stroke, substance abuse, multiple sclerosis, meningitis and/or polio.

    AZA’s Care Managers' expertise also extends to clients with complex mental illnesses such as depression, bipolar disorder, substance abuse and schizophrenia.
