    Our goal is to improve an elder’s quality of life through exceptional care.

    We provide care to older adults in their homes, partnering with our clients to improve their quality of life through exceptional care and care management.

    We offer Three Levels of Care: Companion Care, Personal Care and Specialty Care. All Levels of Care are delivered on an hourly or live-in basis.

    • Basic complimentary Geriatric Care Management (GCM) built into our home care

    • Delivery of care by elder care professionals, from caregivers to management, who are passionate about providing excellent care
    About My Services
    Care Management

    We offer comprehensive Geriatric Care Management (GCM) to compliment each Level of Care as desired. GCM further improves coordination and management of an elder’s care and includes: weekly home visits, short and long-term care planning, cost of care analysis and doctor and clinician consults.

    Companion Care

    Our entry level of care is designed for the elder who has just begun to need help, but requires assistance performing basic functions. Companion Care includes all caregiver tasks with the exception of assistance with the activities of daily living (ADLs) and medications.

    Personal Care

    Our second level of care is designed for the elder who needs assistance performing basic functions. The focus of this care is to support an elder with respect and dignity. Personal Care includes Companion Care and help with activities of daily living (ADLs) and medications.

    Specialty Care

    Our third level of care is designed for the elder with Alzheimer’s/Dementia or significant memory impairment. Specialty Care includes all levels of care and:

    Client & Visitor Activities Planning
    Validation & Reminiscence Exercises

    Our management team brings years of business, healthcare and geriatric care management experience to Sesen Elder Care to provide excellent care to seniors in the Bay Area. Our care combines geriatric care management, excellent caregivers and innovative operations management to provide uniquely tailored, affordable home care to our clients.

    Each of our experienced caregivers have successfully completed an in-depth series of interviews, testing, comprehensive background, drug, health screenings and reference checks before they become a Sesen Elder Care employee. Each caregiver is fully insured and must:

    • Be a licensed CNA (Certified Nursing Assistant) in good standing/have a minimum of three years professional elder caregiving experience
    • Be CPR/First Aid Certified
    • Undergo training at the CNA CEU level on an on-going basis
    • Provide reliable transportation
