    We are Geriatric Care managers offering services and support that are designed to create a positive, holistic environment for aging, remove stress from families and create peace of mind.

    Our goal is to create support that allows elders to live to their full capable independence, while offering their children supporting services that ease the complex demands of work and family.

    We design and implement basic or comprehensive care plans that consider the medical, social, spiritual, legal and financial needs of our clients. From crisis management to formal assessment for care planning creation, their is no task to small or complex for us to help you navigate.

    We have researched and established relationships with the best resources and support systems in the Portland Area to compliment our Care Management Planning and Navigation Services.
    Experience And Credentials
    Susan McCarty is a Gerontologist who specializes in the needs of the oldest of the old. In addition to her professional experience supporting aging adults and their loved ones, she brings her own personal experience of caring for her father with Alzheimer's disease long distance for many years. When considering aging, Susan focuses on moving the discussion of aging from “age as loss” to an emphasis on the gifts of aging. She has a gift for story and legacy building.

    Mary Jo is a Gerontologist with certifications as an Aging-in-Place Specialist (CAPS) and Spiritual Director and specializes in supporting the Boomer Children's complex work and family demands when caring for parents. She brings her own personal experience of caring for her mother with Lewy Body Dementia in addition to her professional finance and care management business skills. Mary Jo's has a gift for generational needs and the spiritual needs of our elders.

    Fees And Payment Sources Accepted
    Our fees range from $85 - $125 per hour depending on the service request. We also offer subscription based monthly long term rates.

    We accept credit card, cash and check payments. Long term support is billed on bi-monthly basis. One time service or short term support is paid at time of service.

    We are a fee for service practice. In making referrals to clients we accept no compensation from providers we refer to and act only on the behalf of our clients best interest.

    Hours of Service
    We offer our consultation and planning services between 8am-5pm, Monday through Friday. Special arrangements can be made for evening and weekend sessions. Implemented care plans vary in support functions and the time of day required. These plans are executed as needed and agreed upon by the client and GCM.
