    Paul Battle Associates provides Geriatric Care Management, Case Management and home nurse practitioner visits to persons with cognitive impairments, physical disabilities and chronic health problems. Our services maximize the client’s health and independence, preserve the client's dignity, promote a safe environment, help prevent crises and support the entire family with trusted, objective, professional and experienced care.

    About My Services
    Geriatric Care Management as a holistic, client-centered approach to caring for older adults or disabled persons. The goal is to improve the client’s quality of life and reduce family stress. Geriatric Care Management is a specialized type of Case Management.

    At Paul Battle Associates we will develop a personalized care plan that may include:
    • Acting as a liaison to families at a distance, overseeing care and alerting them to problems
    • Aging in Place support and coordination of services to help the client continue to live safely in their home
    • Assessment and monitoring including medical, psychosocial, behavioral and spiritual needs
    • Care Coordination including attending physician visits, communication with and between providers and coordination of the treatment plan with a focus on continuity of care
    • Caregiver coaching
    • Health care planning to assist clients in attaining their maximum functional potential and quality of life goals
    • Health education and enhanced health literacy of the client, family and caregivers
    • Home Nurse Practitioner visits for treatment of some urgent care problems
    • Home safety assessment and home modification coordination
    • Medical management
    • Durable medical equipment coordination
    • Medication management

    • Personal and compassionate care focused on the individual

    Conditions I Am Unable To Treat
    While John and Susan Battle are both Board Certified Nurse Practitioners, at Paul Battle Associates they do not provide their clients with primary care. We want each of our clients to have a primary care provider. We work closely with our client's primary care provider and specialists. We help coordinate care and communication between your health care providers. We do not change your physician's medical treatment plan.

    Experience And Credentials
    Paul Battle Associates has the only Certified Geriatric Care Manager is Central Oregon. Paul Battle Associates is owned and operated by John and Susan Battle.

    Susan Battle, MSN, RN, PHN, CCM, GCM, FNP-BC

    Ms. Battle has over 30 years of nursing experience. She graduated with her Bachelor of Science in Nursing from the University of San Francisco and Master of Science in Nursing from the University of California San Francisco. She is licensed as a Registered Nurse, Public Health Nurse and Family Nurse Practitioner with prescriptive authority in California and Oregon. She is Board Certified as a Case Manager and Family Nurse Practitioner and certified as a Geriatric Care Manager.

    Early in her career, Ms. Battle worked in community health care providing skilled nursing services in the home, case management, discharge planning and hospice care. Later in her career as a credentialed member of the medical staff at Tuba City Regional Health Care Center, Ms. Battle provided primary care in the Department of Family Medicine and was the Chairperson for the Pain Management Committee. On the Navajo reservation, Ms. Battle often treated multiple generations of an extended family and through education was able to improve the health status of the entire family. She has provided primary care as a Family Nurse Practitioner at several rural clinics including the La Pine Community Health Center.

    Ms. Battle grew up in Iowa and Arizona. She is married with five adult daughters and three grandchildren. She enjoys quilting, photography, hiking and sailing. Ms. Battle's father has Alzheimer's Disease.

    She volunteers as a Family Nurse Practitioner at the Volunteers In Medicine Clinic of the Cascades in Bend, Oregon. At Paul Battle Associates, Ms. Battle provides case management, life care planning and geriatric care management along with home nurse practitioner visits.

    John Battle, MSN, RN, PHN, ANP-BC

    Mr. Battle grew up in Western Montana. He attended Santa Clara University and earned a Bachelor of Science in Electrical Engineering and a Master of Science in Computer Science. He worked for a number of years in high technology before returning to his passion for health care. He earned a Master of Science in Nursing from the University of California San Francisco. Mr. Battle is Board Certified as an Adult Nurse Practitioner with prescriptive authority in Oregon and California, a Registered Nurse and a Public Health Nurse.

    Mr. Battle served the Navajo, Hopi and Southern Paiute tribes in Northern Arizona where he provided primary care in the Department of Internal Medicine and provided urgent care services in the same day walk-in clinic at Tuba City Regional Health Care Center. He has experience treating patients with complex chronic illnesses and multiple comorbidities. In 2010 he and his wife purchased their home in Bend, Oregon. He practiced in occupational health and continued providing primary care for medically complex patients in Central Oregon with an emphasis on disease management and prevention, health education and addressing the well being of the entire family. He volunteers with Volunteers in Medicine Clinic of the Cascades treating patients with limited access to health care. At Paul Battle Associates, he advocates and coordinates the care of seniors and people with disabilities or who have suffered catastrophic injuries. He provides in-home Nurse Practitioner care of appropriate conditions for his regular patients.
    Fees And Payment Sources Accepted
    We provide an initial meet and greet contact at no cost.

    Cost of initial assessment is $200, then services are $95 per hour. Providing the right care at the right time helps contain costs, save money and reduce stress.

    Paul Battle Associates' usual hours of operation are 8 AM to 5 PM Monday through Friday. Services are available during other hours as needed and for emergencies.
