    About My Business
    MN Senior Care Partners is a Geriatric Care Management company helping seniors, adults with disabilities and their families navigate the complex senior industry.
    About My Services
    Kristin can help families with an initial assessment to identify the main issues / concerns surrounding an older loved one. Then a care plan can be created and referrals given to provide the family with an appropriate plan of action. Additional support services can be provided to find home care agencies, identify the right housing options, and figure out how to pay for services and how to find supplemental assistance to support not only the senior but the family as well.

    Kristin Smith has over 20 years of non-profit / social service experience and has assisted hundreds of families in the Twin Cities area.

    She is an active member with the National Association of Geriatric Care Managers, Minneapolis Area Senior Workers Association and several other professional networking groups in the Twin Cities area.
