Long Island Alzheimer's and Dementia Center (LIAD Center) is driven by a mission to improve the quality of life for those living with Alzheimer’s disease and related forms of dementia and their caregivers through the provision of day programs for all stages of Alzheimer’s disease from pre-diagnosis to end of life; caregiver support groups and training; in-home respite solutions; transportation options; resources and referral support; and individual counseling.
Founded in Port Washington, New York in 1988—when we were known as the Long Island Alzheimer's Foundation—LIAD Center has provided innovative community-based support services from pre-diagnosis through end-of-life for individuals with Alzheimer’s disease (AD) and related dementias and their families. As one of the longest-operating organizations on Long Island for those with AD, we have long been a resource for families affected by memory loss, providing daily in-person cognitive stimulation, exercise, and social interaction activities at our centrally located, fully accessible center in Westbury, New York. Our programs foster the independence, dignity, well-being and safety of individuals with dementia, aiding in the prevention of premature nursing home placement, and offering hope and help for families grappling with degenerative memory loss. Our Caregiver Support Programs-including our in-Home Respite Program; Individual Counseling and Caregiver Trainings-are designed to provide caregivers with the time, support and skills they need to mitigate the stresses of providing around-the-clock care for their loved one who has been diagnosed with cognitive loss.

Basic information about non-licensed providers is collected from private sources. Information is collected when providers complete a free Provider Profile, outlining the care and services they offer. HealthlinkDimensions is solely responsible for this activity. Additionally, program information from the Alzheimer’s Association and AARP is compiled.
