Anticipating the Transition to a Care Setting: Placing a loved one in a care facility is one of the most challenging and emotional choices a family can face on the caregiving journey. This presentation provides supportive discussion about considering a care home and affirms the caregiver's experience navigating these steps. We will also focus on need-to-know information for exploring care home options and strategies for a successful transition. Alzheimer's Awareness: This presentation provides a brief overview of Alzheimer’s disease, dementia and the Alzheimer’s Association services. Topics include the impact of Alzheimer’s, risk factors, warning signs and how others can join the fight against the disease.
