Making those person-centered care plans meaningful, individuals should design care plans that will identify problems, determine goals, plan interventions needed to help the person meet their goals. By re-evaluating the outcomes of interventions, developing a plan of care specifically suitable for the person living with a dementia diagnosis which reflects his/her current status.
